Here you can download the official application form for Rebuilding Dreams. This is your chance to share your family’s story! You can also use this form to nominate a family that you know.
The application form is a PDF file that you can fill out on your computer and send in by email. In order to properly view the file and fill out the form, please make sure you have the latest Adobe Acrobat® Reader software installed on your computer. It is free and available to download below:
Step 1: Download & install Adobe Acrobat® Reader software
Step 2: Download the application form
Right-click the link below and select “Save target as…” or “Save link as…” to save the file to your computer.

Step 3: Open the application file with Adobe Acrobat® Reader and type in your responses.
Step 4: Apply digital signatures to the form where indicated.

When you click on these areas, you will be asked to create a Digital ID. Once you have created your Digital ID, you can sign the application form digitally.
Step 5: Save the file and email it to us along with photos of all the family members in the household.
